يسعى مستشفى الدكتور سليمان الحبيب (الرياض، السعودية) إلى توظيف ممرض/ة عناية بالجروح متخصص/ة لتقديم رعاية شاملة للمرضى بناءً على الممارسات القائمة على الأدلة.

نبذة عن الوظيفة

تقديم رعاية شاملة للجروح للمرضى باستخدام الممارسات القائمة على الأدلة. تشمل المسؤولية التقييم والملاحظة وتوفير مستوى آمن من رعاية المرضى وتقييم نتائجها. تقديم تثقيف للمريض والعائلة والموظفين حول إدارة الجروح، والوقاية من تقرحات الضغط، وأسطح الدعم، واختيار الضمادات والعلاجات الأخرى، ويكون مصدرًا مرجعيًا للأطباء وفريق الجروح والموظفين السريريين. المشاركة في خطة تحسين الأداء في المنشأة.

المهام والمسؤوليات

  • تقييم الاحتياجات التعليمية للطاقم التمريضي والتعاون مع رئيسة الممرضين/التعليم التمريضي لتعزيز معرفة الموظفين بالتعامل مع الأفراد المعرضين لخطر الإصابة بالتقرحات أو المصابين بها بناءً على الممارسة السريرية.
  • إجراء تقييم وإعادة تقييم وإدارة الجروح لجميع المرضى وفقًا للسياسة، ويشمل ذلك حديثي الولادة والأطفال والمسنين والبالغين بشكل عام.
  • إظهار المعرفة بمبادئ النمو والتطور على مدى العمر والمهارات اللازمة لتقديم رعاية مناسبة للفئة العمرية للمرضى.
  • إظهار معرفة شاملة بنظام تقييم الجروح وتصنيفها.
  • وضع وتحديث معايير الرعاية والكفاءات والسياسات والإجراءات المتعلقة ببرنامج العناية بالجروح بما في ذلك بروتوكولات رعاية المرضى وأدوات التوثيق.
  • تقديم التدريب والدعم لأفضل الممارسات التمريضية المحدثة في العناية بالجروح.
  • مساعدة رئيسات الممرضين/الممرضات المسؤولات في تحديد الحاجة إلى تغيير الممارسة الحالية بناءً على المواد المتاحة للعناية بالجروح والمساعدة في تنفيذ التغييرات المطلوبة وفقًا للاحتياجات ومتطلبات الرعاية التمريضية بالتنسيق مع معايير وتوجيهات مجموعة حبيب الطبية.
  • مراقبة معدلات تقرحات الضغط المكتسبة في المستشفى وقيادة الفريق لإجراء مسح انتشار تقرحات الضغط على الأقل كل ثلاثة أشهر.
  • استشارة المتخصصين/الأقسام الصحية الأخرى حسب الاقتضاء لتوفير نهج متعدد التخصصات بناءً على احتياجات المريض.
  • مراجعة خطة الرعاية بناءً على استجابة المريض للعلاج وتقييم الفعالية الكلية للخطة.
  • مراقبة الامتثال لسياسة العناية بالجروح.
  • المشاركة في تثقيف وتقييم المنتجات والمعدات والتقنيات والعمليات الجديدة لتحسين إدارة الجروح.
  • المشاركة في البرامج التعليمية والدورات التدريبية وورش العمل ذات الصلة لزيادة الكفاءة المهنية وتلبية الاحتياجات والأهداف الشخصية.
  • تقديم تثقيف للمريض والعائلة والموظفين حول إدارة الجروح والوقاية من تقرحات الضغط وأسطح الدعم واختيار الضمادات والعلاجات المساعدة الأخرى.
  • تقديم تدريب أساسي في العناية بالجروح وتوجيه لجميع الموظفين التمريضيين الجدد.
  • الإبلاغ عن الممارسات غير الآمنة أو عدم الكفاءة المهنية أو سوء السلوك المهني أو عدم القدرة أو عدم اللياقة لممارسة المهنة لأي عضو في فريق الرعاية الصحية من خلال القنوات المناسبة.
  • تقديم ملاحظات بناءة لأعضاء فريق الرعاية الصحية في الوقت المناسب.
  • وضع خطة تعليمية بناءً على احتياجات الموظفين التعليمية المحددة وتقييم فعالية التعلم.
  • تسهيل تنفيذ السياسات والبرامج التعليمية الجديدة أو المعدلة.
  • إظهار الأخلاقيات المهنية والتعليمية المتعلقة بالممارسة الطبية وسرية المريض.
  • المساعدة في تحديد احتياجات تثقيف المريض/العائلة والمشاركة في تطوير مواد التثقيف الخاصة بالمريض/العائلة.
  • تقديم تثقيف للمريض والعائلة حسب الاحتياجات.
  • المشاركة في مبادرات الرعاية المتمحورة حول الشخص التي تنفذها مجموعة حبيب الطبية.
  • إثراء تجربة المريض بالتعاطف والاحترام والكرامة.
  • أداء المهام والواجبات الأخرى المطبقة التي يتم تكليفه بها ضمن نطاق معرفته ومهاراته وقدراته.

الشروط والمتطلبات

  • درجة البكالوريوس أو الماجستير (يفضل تخصص التمريض أو التثقيف الصحي).
  • خبرة لا تقل عن 5 سنوات في نفس التخصص.
  • تسجيل ساري المفعول من الهيئة السعودية للتخصصات الصحية.

Description

Job Purpose / Objective

Provides comprehensive wound care to patients using evidence-based practice. Responsible for assessment, observation, and provision of a safe standard of patient care and evaluation of its results. Provides patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies, is a resource person for physicians wound team and clinical staff. Participates in performance improvement plan of the organization.

Key Responsibilities / Accountabilities

  • Assesses the learning needs of nursing staff and collaborates with the Head Nurse Nursing Education to enhance staff knowledge on dealing with individuals at risk for developing ulcers or with existing ulceration based on clinical practice.
  • Performs wound assessment, reassessment and management for all patients as per policy. This includes neonates, pediatric, geriatric, and the general population.
  • Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population being served.
  • Demonstrates thorough knowledge of wound assessment and staging system.
  • Establishes and updates standards of care, competencies, policies and procedures about wound care program to include patient care protocols and documentation tools.
  • Provide training and support in the provision of best updated nursing practice on wound care.
  • Assists the Head/ In-charge Nurses in identifying the need for change in current practice based on the available wound care materials and assists with the implementation of required changes based on needs and nursing care requirements in conjunction with HMG standards and guidelines.
  • Monitors hospital acquired pressure rates and leads the team for conducting Pressure Ulcer Prevalence Survey at least quarterly.
  • Consults other healthcare professionals/departments as appropriate to provide interdisciplinary approach specifically based on the patient’s needs.
  • Revises plan of care as indicated by the patient's response to treatment and evaluates overall plan for effectiveness.
  • Monitors compliance with wound care policy.
  • Participates in the education for and the evaluation of new products, equipment, technology and system processes to improve wound care management.
  • Participates in relevant education programs, in-services and workshops to increase professional competency and meet personal needs and goals.
  • Provides patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selection and other adjunct therapies.
  • Provides basic wound care training and orientation to all newly hired nursing staff.
  • Reports unsafe practice, professional incompetence, professional misconduct and incapacity or unfitness to practice of any health care team member through appropriate channels.
  • Provides constructive feedback to members of the health care team in a timely manner.
  • Formulates a teaching plan based upon identified staff learning needs and evaluates effectiveness of learning.
  • Facilitates the implementation of new or revised policies and educational programs.
  • Demonstrates professional and educational ethics related to medical practice and patient confidentiality.
  • Assists in identifying patient/family education needs and participates in the development of patient/family education materials.
  • Provide patient and family education as per the needs.
  • Participate in person-centered care initiatives undertaken by HMG.
  • Enrich patient experience with compassion, respect and dignity.
  • Performs other applicable task and duties assigned within the realm of his/ her knowledge, skills and abilities.

Requirements

  • Education/ Professional Qualification Bachelor’s degree or Master’s Degree Professional - Preferably Nursing / Health Education
  • Experience Minimum of at least 5 years working experience in same specialty
  • Professional Licensing / Certification / Training Valid registration of Saudi Council
