نبذة عن الوظيفة

صرف الأدوية الموصوفة من قبل أطباء مرخصين مع الالتزام بأعلى معايير السلامة والجودة، وضمان عملية سلسة وسريعة تساهم في تحقيق أعلى مستويات رضا المرضى.

المهام والمسؤوليات

  • مراجعة وتفسير وتوضيح أوامر الأدوية من الأطباء، والتحقق من جميع الجرعات والمدة، وفحص التداخلات الدوائية المحتملة والتواصل مع الطبيب في حال وجود أي غموض أو استفسار.
  • الإشراف على تحضير الأدوية المعتمدة، وإجراء مراجعة مزدوجة والتأكد من وضع العلامات المناسبة للأدوية المحضرة ثم صرفها.
  • حساب كمية الدواء المطلوب صرفها للوصفة الطبية بناءً على وزن الجسم وعمر المريض.
  • إدخال الوصفات الخارجية أو الأوامر في نظام CPOE الخاص بنظام معلومات فقيه الصحي (Fakeeh Care HIS) قبل الصرف.
  • ضمان استدعاء المرضى إلى نوافذ الصرف بسرعة وفي الوقت المناسب، والعمل على تقليل وقت الانتظار إلى أدنى حد ممكن.
  • تقديم المشورة للمرضى ومقدمي الرعاية حول الاستخدام الآمن والسليم للأدوية، والتخزين، والتفاعلات، وغير ذلك.
  • اتباع جميع إجراءات سلامة الأدوية عند صرفها.
  • الحفاظ على مراقبة المخزون وأمن جميع الأدوية، والتسجيل والتخزين الآمن للمواد المخدرة والخاضعة للرقابة.
  • تحديد الأدوية التي شارفت صلاحيتها على الانتهاء والتعامل معها وفقاً لسياسات فقيه للرعاية الصحية، وعدم صرف أي دواء انتهت صلاحيته.
  • إتمام تحصيل تكاليف الأدوية المصروفة من المرضى بشكل صحيح.
  • إظهار المعرفة بالتخزين والتعامل السليم مع المواد الخطرة في مكان العمل، بالإضافة إلى إجراءات الإسعافات الأولية اللازمة للتعرض العرضي.
  • إظهار المعرفة والمهارات في استخدام قائمة الأدوية المعتمدة (formulary)، وتحديد البدائل الجنيسة، ودواعي الاستخدام، ودور حدود الخصم وتطبيق قيود كمية الوصفات.
  • الممارسة والامتثال الكامل لمعايير مكافحة العدوى والوقاية منها الحالية عند التعامل مع الأدوية وتحضير محاليل الوريد (IV) والتغذية الوريدية (TPN).
  • المساهمة في تدريب وإعداد الصيادلة الجدد والطلاب والمتدربين.
  • إظهار المعرفة بالأدوية التي تصرف بدون وصفة طبية (OTC).
  • الإبلاغ إلى المشرف/المدير عن أي خطأ دوائي أو تفاعل دوائي ضار (ADR) وتسجيله في نظام OVR.
  • حضور برامج التعليم المستمر للحفاظ على الخبرة الصيدلانية والتراخيص المطلوبة.
  • ضمان التسجيل في برنامج التعليم المستمر فيما يتعلق بممارسات إدارة الدواء، والأدوية عالية الخطورة، والأخطاء الدوائية وغيرها.
  • تثقيف المرضى وعائلاتهم حول الحالات الصحية والعلاجات والتدابير الوقائية إذا لزم الأمر.
  • جميع موظفي فقيه للرعاية الصحية مسؤولون عن التحسين المستمر، ويشمل ذلك أداء جميع المهام بطريقة تدعم قيم الرعاية المتمحورة حول الشخص، والمساهمة الفعالة في مبادرات التحسين المستمر ضمن نطاق الدور، والالتزام ببروتوكولات السلامة، والحفاظ على السرية التامة، والامتثال لسياسات الأمن السيبراني، والالتزام بقواعد السلوك والمعايير الأخلاقية، وإتمام التعليم الإلزامي قبل شهر واحد على الأقل من انتهاء صلاحيته.

الشروط والمتطلبات

  • درجة البكالوريوس في الصيدلة.
  • خبرة من 0 إلى 2 سنة في بيئة الصيدلية بالمستشفى.
  • إجادة اللغة الإنجليزية تحدثاً وكتابة.
  • رخصة مزاولة المهنة من الهيئة التنظيمية الصحية الإقليمية (مثل SCFHS أو DHA).

المهارات المطلوبة

  • مهارات حاسوبية جيدة في استخدام برامج مثل Microsoft Word و Excel.
  • السعي النشط لتطوير المهارات المهنية من خلال المشاركة في التدريب والخبرات المناسبة.

Description

Job Purpose:

Dispenses medications ordered by privileged physicians while observing at all times highest standards of safety and quality and ensuring prompt seamless process that is conducive to highest patient satisfaction.

Key Responsibilities And Duties

  • Reviews, interprets, clarifies doctor’s medications orders and verifies all orders doses, duration and checks for potential drug-to-drug interaction and clinical indication. Communicates with ordering doctor in case of any ambiguity or concern.
  • Oversees the preparation of the verified medications, performs double-checking and review of proper labeling for the prepared medications and then dispenses.
  • Calculates the amount of the drug product to dispense for a prescription based upon body weight and patient age.
  • Performs computer entry of outside prescriptions or orders into CPOE of Fakeeh Care HIS before dispensing.
  • Ensures prompt and timely calling of patients to dispensing windows and aims at reducing patients waiting time to the minimum possible.
  • Counsels the patients and caregivers on the proper & safe use of medication, storage, and interactions, etc.
  • Follows all medication safety procedures when dispensing medications.
  • Maintains inventory control & security of all drugs, record keeping and secured storage of narcotics and controlled medications.
  • Identifies and appropriately handles the nearly expired medications according to Fakeeh Care policies and NEVER dispenses a medication beyond its expiry date.
  • Properly completes charging of patients for dispensed medications.
  • Demonstrates knowledge of proper storage & handling of hazardous materials found in her/him workplace, as well as first aid measures necessary for accidental exposure.
  • Demonstrates knowledge & skills in utilizing formulary, identifications of generic substitution, indication and the role of deductible limits and application of prescription quantity limitations.
  • Practices and fully complies with current infection prevention and control standards when handling medications and preparing IV and TPN formulas.
  • Contributes to training and induction orientation for newly hired pharmacists and students and trainees.
  • Demonstrates knowledge of commonly used over the counter (OTC) drugs.
  • Reports to pharmacy supervisor/ manager and enters on OVR any encountered medication error or ADR.
  • Attends continuing education programs to maintain expertise in pharmacy and required licensure.
  • Ensures Enrollment in continuous education program regarding medication management practices, high risk medications, medication errors and others.
  • Educates patients and their families about health conditions, treatments and preventative measures if applicable.

Requirements

Skills and Abilities:

  • Competent computer skills with software applications to include Microsoft Word, Excel.
  • Actively strive to upgrade professional skills through engaging in appropriate professional training and experience.

Experience

0 - 2 years of experience in hospital pharmacy setting.

Education

Bachelor’s Degree of Pharmacy.

Language

Good command of oral and written English.

Licenses / Certifications

License for practice as per the regional health regulatory authority e.g. (SCFHS / DHA).
