Lead the end-to-end operations, execution, delivery, service, and outcomes of all Partnership and referral activities, including, but not limited to, Home Clinical Visits, Labs, PT, Vaccinations, Imaging, etc. across all connected care channels and patient touchpoints
Appointment and Data Management
- Efficiently schedule home visits using data analysis.
- Analyze appointment data for trends and improvements
Quality and Result Management
- Collaborate for quality checks with third-party providers.
- Oversee lab result delivery within KPIs.
Patient-Centric Approach:
- Maintain a patient-centric focus across activities
Operational Excellence
Continuous Improvement:
- Assess and improve care quality across all channels.
- Implement and monitor quality assurance measures.
- Foster partnerships for referrals and care continuum
- Collaborate for data-driven system solutions
Collaboration and Establish key performance indicators
- Maintain a patient-centric focus throughout all activities, meeting patient needs and preferences.
- Establish key performance indicators (KPIs), and SLAs to measure signed-off outcomes and align expectations across all connected care touchpoints
Service Escalation
- Act as a point of contact for addressing and escalating any service failures or concerns.
- Collaborate with the team to implement corrective actions and prevent future issues
Medical Reports Explanation &Digitalization and Data-Driven Solutions
- Interpret and explain medical reports to patients and their families clearly and compassionately when necessary
- Collaborate with the IT and Digital Factory teams to enhance and finalize data-driven system solutions for both laboratory services and home-based lab bookings.
- Leverage data analysis to contribute to the digitalization of the home healthcare journey, ensuring data-driven decision-making
- Bachelor’s degree in medicine and Surgery , Healthcare Management ( Preferable have master degree )
- Proven experience in home health care management or a similar leadership role.
- Strong understanding of medical Guidelines and procedures.
- Excellent interpersonal and communication skills
- Ability to analyse and interpret data for informed decision-making.
- Knowledge of healthcare regulations and compliance
- Up to 6 years clinical experience is required
