تعلن شركة فقيه للرعاية الصحية عن توفر وظيفة أخصائي تروية قلبية وعائية (Cardiovascular Perfusionist) في الرياض - حي الياسمين.

المهام والمسؤوليات

  • تشغيل جهاز القلب والرئة وتوفير التروية القلبية للمرضى أثناء العمليات الجراحية القلبية وغير القلبية.
  • مراجعة التاريخ الطبي للمريض بما في ذلك المرحلة المرضية والعدوى ونوع الجراحة المخطط لها.
  • اتباع بروتوكولات التهيئة (Priming) للبالغين والأطفال والرضع، وتجهيز دائرة التروية بالسوائل وفقاً للبروتوكول أو بالتشاور مع الجراح / طبيب التخدير.
  • استخدام الأدوية في محاليل التهيئة حسب البروتوكول للبالغين والأطفال والرضع، وإضافة أدوية أخرى بجرعاتها فقط بناءً على advice من طبيب التخدير.
  • تجميع وتجهيز المعدات الإضافية حسب متطلبات الجراحة أو رعاية المريض قبل وأثناء وبعد المجازة (مثل IABP).
  • التأكد من اختيار المستهلكات المناسبة حسب عمر المريض (بالغ، طفل، رضيع) مع مراعاة التكلفة دون المساس بالأداء والسلامة.
  • التحقق من تواريخ التعقيم وتواريخ انتهاء صلاحية المستهلكات والسوائل والأدوية قبل الاستخدام.
  • التحقق من سلامة المواد المستهلكة المستخدمة من التلف والفحص.
  • تجميع دائرة المجازة القلبية الرئوية تحت ظروف وإجراءات تعقيم صارمة.
  • المسؤولية عن الحماية من أي ضرر أثناء إجراء التروية.
  • التخلص من النفايات والمستهلكات بعد كل استخدام بالطريقة الصحيحة واتباع أكياس التخلص المرمزة بالألوان للأمراض التي تهدد الحياة مثل HIV والتهاب الكبد.
  • ضمان سلامة المريض أثناء الإجراء باتباع خطوات السلامة التي تمنع دخول الصمات الدقيقة والكبيرة في الدائرة وعدم السماح لجهاز الأكسجة بأن يفرغ.
  • الحفاظ على تدفق دم مناسب للمريض أثناء إجراء التروية، وحساب التدفق المناسب حسب البروتوكول والمحافظة عليه طوال المجازة.
  • الحفاظ على ضغط دم مناسب للمريض أثناء المجازة القلبية الرئوية ضمن حدود الدائرة والإجراء الجراحي والقيود الفسيولوجية للمريض، مع عناية خاصة لمرضى الأمراض الدماغية والكلوية.
  • الحفاظ على أكسجة مناسبة للدم أثناء المجازة القلبية الرئوية بتقييم حالة المريض المسبقة ونوع الجراحة لاختيار جهاز الأكسجة المناسب مع مراعاة سن المريض.
  • تشغيل مضخات الشفط والأجهزة التي تزيل الدم من مجال العملية أو تخفف الضغط عن حجرات القلب المختلفة، مع المعرفة والفهم لأهمية صدمة الدم بهذه الأجهزة.
  • الحفاظ على درجات حرارة الدم وبالتالي درجة حرارة جسم المريض، والاستخدام الآمن لتدرجات الحرارة المناسبة عند التبريد أو إعادة التدفئة واستخدام الأجهزة المناسبة لتحقيق ذلك بأمان.
  • مراقبة المعايير المختلفة والأجهزة المدمجة لسلامة المريض المطلقة: تخطيط القلب، ضغط الدم الشرياني، الضغط الوريدي المركزي والضغط الأذيني الأيسر، ضغط القنية الأبهرية، درجات حرارة المريض (المستقيم والجلد)، درجات حرارة الدم الوريدي والشرياني، تدفق الأكسجين، معدل تدفق الدم، مؤشر القلب، درجة حرارة حماية عضلة القلب، وقت المشبك الأبهري، وقت المجازة، وقت الهيبارين، مختبر ACT (قبل المجازة)، مضخة البالون داخل الأبهر وجهاز إنقاذ الخلايا، وتروية الكلى لزراعة الكلى.
  • تثقيف المرضى وعائلاتهم حول الحالات الصحية والعلاجات والإجراءات الوقائية إذا كان ذلك مناسباً.
  • أخصائي تروية أول: مراقبة المخزون ومستلزمات جراحة القلب المفتوح يومياً والتحكم في عملية توريد المستهلكات اللازمة لإجراء العمليات الجراحية.
  • إعداد جدول العمل اليومي للفريق، وجدول المناوبات لأخصائيي التروية، وخطة الإجازات وورش العمل التدريبية.
  • إعداد قائمة المعدات اللازمة لأداء عمليات جراحة القلب المفتوح، والتواصل مع الشركات لأي تسليم أو دعم فني.
  • إدارة قاعدة بيانات القسم للإحصائيات الإجرائية والبحوث وأي بيانات/معلومات مطلوبة تتعلق بالقسم.

الشروط والمتطلبات

  • خبرة لا تقل عن 5 سنوات في مجال التروية القلبية مع القدرة على العمل بشكل فردي وإجراء التروية لمرضى بالغين وأطفال ورضع أثناء جراحة القلب المفتوح بدقة وكفاءة.
  • معرفة خلفية ممتازة بالمصطلحات الطبية ومعرفة عامة بتشريح القلب والرئتين والأوعية الكبرى وكذلك علم وظائف الأعضاء البشري.
  • المؤهل العلمي: بكالوريوس في العلوم، أو دراسات عليا، أو طبيب عام، أو دبلوم أو دبلوم عالي في تقنية التروية من جامعة أو مستشفى معترف به.
  • اللغة: إجادة اللغتين الإنجليزية والعربية (يفضل).
  • الشهادات والتراخيص: ترخيص ممارسة المهنة من الهيئة الصحية التنظيمية الإقليمية (مثل الهيئة السعودية للتخصصات الصحية / هيئة الصحة بدبي)، بالإضافة إلى شهادة BLS و ACLS.

Key Responsibilities and Duties:

  • The primary duty of a Cardiovascular Perfusionist - Senior Cardiovascular Perfusionist is the operations of the

Heart-Lung machine and provides cardiovascular perfusion for a patient cardiac or non-cardiac surgery.

  • Reviews patient’s medical history such as pathological stage and infections, type of surgery planned for

conditions and diagnosis.

  • Follows priming protocols for adults, pediatrics and infants. Priming the circuit with fluids as per protocol or

in consultation with surgeon/anesthetist for any special care.

  • Uses of drugs in priming solutions as per protocol for adults pediatric and infants. Addition of other drugs

and their doses only on the advice of the anesthetist.

  • Assembles and prepares additional equipment as per the requirement of the surgery or patient care, before,

after, during or after bypasses namely IABP etc.

  • Ensures what disposables to be used depending on adult, pediatric, or infant patients, keeping in mind cost

containment without compromising on performance and safety of perfusion circuit.

  • Checks the dates of sterilizations, date of expiry of disposables, fluids and drugs before using them.
  • Checks the integrity of the disposable materials to be used for damage and checks.
  • Assembles the cardiopulmonary bypass circuit under strict sterile conditions care and precautions.

Cardiovascular Perfusionist - Senior Cardiovascular Perfusionist is further responsible for protection against

any damage during perfusion procedure.

  • Disposes the waste and disposables after every use in a proper manner and following color coding disposable

bags for life threatening diseases like HIV, hepatitis etc.

  • Ensures the safety of the patient undergoing procedure by following safety steps avoiding entry of micro and

macro emboli in the circuit and not allowing the oxygenating device to be empty.

  • Maintains adequate blood flow for the patent undergoing perfusion procedure. Calculating proper blood flow

according to the protocol and maintaining throughout bypass.

  • Maintains adequate blood pressure for the patient undergoing CPB within limitations of CPB circuit and

surgical procedure and physiological limitations of the patient, with extra care for the patients with cerebral

and renal diseases.

  • Maintains adequate oxygenation of blood during CPB by assessing patients’ pre-existing condition and type of

surgery schedule to choose proper and suitable oxygenating device keeping in mind the age of the patient

whether adult, pediatric or infant.

  • Operates suction pumps and devices that remove blood from the operating field or venting and

decompressing various chambers of the heart. Having knowledge and understanding the significance of

blood trauma by these devices.

  • Maintains and regulates blood temperatures and thereby patient’s body temperature. Safe use of adequate

temperature gradients when cooling or re-warming and using the proper devices to accomplish the

procedure safely.

  • Monitors various parameters and in-line devices for the patient's absolute safety:
  • Electrocardiogram.
  • Arterial Blood pressure.
  • Central venous and LA pressures.
  • Aortic cannula pressure.
  • Patient temperatures rectal and skin.
  • Venous blood and arterial blood temperatures.
  • Oxygen flow.
  • Blood flow rate.
  • Cardiac Index.
  • Myocardial protection temperature.
  • Aortic cross clamp time.
  • Bypass time.
  • Heparin time.
  • ACT Laboratory (Pre-bypass).
  • Intra-Aortic balloon pump and cell saver. Kidney perfusion for renal transplants etc.
  • Educates patients and their families about health conditions, treatments and preventative measures if

applicable.

  • Duties related to Senior Cardiovascular Perfusionist: - Monitors inventory and open-heart supplies on daily basis and control the supplying process of consumables

needed for surgery procedures. - Prepares the daily staff work assignment, on call schedule for perfusionist, vacation plan and training

workshop. - Prepares the inventory items equipment needs to perform open heart surgical procedures. Communicates

with companies for any delivery or any technical support. - Database management for department statistic for procedure, research and any required data/ information

related to department

Job Requirements

Skills and Abilities: -

Experience

Minimum of 5 years. Be able to perform individually, do perfusion to adult, pediatric

and infant patients during Open Heart Surgery with accuracy and efficiency Excellent

background knowledge of medical terminology and general knowledge of anatomy of

heart, lungs and great vessels and also general human physiology

Education: University Science Graduate, Post Graduate or Basic Medical Doctor or a Diploma or

Post Graduate Diploma in Perfusion Technology from reputable University or Hospital

Language: English & Arabic Preferable

Licenses / Certifications: License for practice as per the regional health regulatory authority e.g. (SCFHS /

DHA), BLS and ACLS
