تعلن فقيه للرعاية الصحية عن توفر وظيفة استشاري طب الأسرة - طب الرعاية التلطيفية في المدينة المنورة.

المهام والمسؤوليات

أداء الواجبات السريرية وفقاً للامتيازات السريرية الممنوحة له/ها.
إجراء تقييم شامل للمرضى (سريري، اجتماعي اقتصادي، نفسي، وظيفي) وتوثيق التقييم في السجلات الطبية.
وضع خطة رعاية للمرضى باستخدام منهج قائم على الأدلة ووفقاً لمعايير الرعاية، وتوثيقها في السجل الطبي.
ضمان تنفيذ خطة رعاية المرضى في الوقت المناسب.
يكون الطبيب المسؤول الأول (MRP) عن المرضى المنومين تحت اسمه/ها.
وضع خطة خروج للمرضى المنومين تحت اسمه/ها في يوم القبول، وإشراك خدمة التخطيط للخروج في أقرب وقت ممكن عند الحاجة.
تقييم المرضى المنومين تحت اسمه/ها مرة واحدة يومياً على الأقل، وتوثيق التقييم اليومي وخطة الرعاية في ملاحظات التقدم اليومية باستخدام نموذج SOAP.
الاستجابة للاستشارات الطبية في الوقت المناسب.
توثيق التدخلات الطبية التي يقوم بها/ها للمرضى في الوقت المناسب.
الالتزام بمعايير فقيه للرعاية الصحية لتوثيق السجلات الطبية.
المسؤولية عن إكمال ملخصات الخروج وتوثيقها للمرضى المنومين تحت اسمه/ها في الوقت المناسب، والتأكد من تقديم تعليمات الخروج والتوعية حول الأدوية المنزلية قبل خروج المرضى.
الالتزام بواجبات مرفق فقيه للرعاية الصحية والاستجابة للمكالمات من المرفق وفقاً لجدول المناوبات القسمي/الوحدة.
حضور اجتماعات القسم والاجتماعات العامة للموظفين والمساهمة بأفكار لتحسين الكفاءة والإنتاجية ورضا المرضى.
المسؤولية عن كتابة التقارير الطبية للمرضى في الوقت المناسب ووفقاً لتنسيق فقيه للرعاية.
المساهمة في جولات التدريس والأنشطة التعليمية بناءً على طلب رئيس القسم الطبي.
توعية المرضى وعائلاتهم حول الحالات الصحية والعلاجات والتدابير الوقائية.
مهام أخرى تُسند ضمن نطاق المسؤولية ومتطلبات الوظيفة.

الشروط والمتطلبات

خبرة: إكمال خبرة وفقاً للهيئة التنظيمية الصحية الإقليمية (كما هو مذكور في التعليم) في مجال التخصص، في مراكز طبية معترف بها.
تعليم: زمالة / بورد.
تراخيص: رخصة ممارسة حسب الهيئة التنظيمية الصحية الإقليمية (مثل SCFHS / DHA)، وشهادة دعم الحياة الأساسي (BLS)، ورخصة متقدمة إن وجدت.

المهارات المطلوبة

القدرة على التواصل بوضوح مع المرضى وعائلاتهم والأطباء والموظفين غير الطبيين.
مهارات استخدام الكمبيوتر (Microsoft).
مهارات تنظيمية جيدة والقدرة على العمل في ظروف ضغط عالية.

About the job

Key Responsibilities And Duties

Performs clinical duties as per his / her assigned clinical privileges.
Assesses his / her patients thoroughly (eg. clinical, socioeconomic, psychological and functional assessments) and documents patients' assessment in patients' medical records.
Develops a plan of care for patients using an evidence-based approach and following standards of care, and documents the plan of care in patients’ medical record.
Ensures that patients’ plan of care that is put by him/her is being implemented in a timely manner.
Becomes the most responsible physician (MRP) for inpatients admitted under his / her name.
For inpatients admitted under his / her name, develops discharge plan on day of admission and involves discharge-planning service as early as possible if warranted.
Assesses inpatients that are admitted under his / her name at least once daily and ensures documentation of daily assessment and plan of care in daily progress notes using SOAP format.
Responds to medical consultations in timely manner.
Documents medical interventions that he / she performs on patients in a timely manner.
Strictly follows Fakeeh care standards for documentation in medical records
Responsible for timely completion and authentication of discharge summaries for patients admitted under his / her name. Also responsible for ensuring that indicated discharge instructions and education on take home medications is done prior to patients discharge.
Attends to Fakeeh Health Care facility duties and responds to calls from the facility as per the departmental /section call schedule.
Attends departmental and general staff meetings and contributes ideas for improving efficiency, productivity and patient satisfaction.
Responsible for writing medical reports of his / her patients in a timely manner
and as per Fakeeh care format
Contributes to teaching rounds and teaching activities as requested by chairperson of medical department
Educates patients and their families about health conditions, treatments and preventative measures.
Other duties as assigned within the scope of responsibility and requirements of the job.

Job Requirements

Skills and Abilities:

Must be able to clearly communicate with patients and their families, physicians
and non-medical staff.
Microsoft computer skills.
Should have good organization skills and be able to work in high stress situations.

Experience

Completion of experience according to the regional health regulatory authority
(as mentioned in education) within the field of specialty, in recognized medical centers.
