صيدلي عيادات خارجية في شركة فقيه للرعاية الصحية، مكة جدة، السعودية.

نبذة عن الوظيفة

صرف الأدوية الموصوفة من قبل الأطباء المصرح لهم مع الالتزام بأعلى معايير السلامة والجودة وضمان عملية سلسة وسريعة تساهم في تحقيق أعلى مستويات رضا المرضى.

المهام والمسؤوليات

  • مراجعة وتفسير وتوضيح أوامر الأدوية من الأطباء، والتحقق من جميع الجرعات والمدة، وفحص التفاعلات الدوائية المحتملة والتأكد من المؤشر السريري، والتواصل مع الطبيب الموصي في حال وجود أي غموض أو مخاوف.
  • الإشراف على تحضير الأدوية التي تم التحقق منها، والقيام بالتدقيق المزدوج ومراجعة وضع العلامات الصحيحة للأدوية المحضرة ثم صرفها.
  • حساب كمية الدواء المطلوب صرفها بناءً على وزن الجسم وعمر المريض.
  • إدخال الوصفات الخارجية أو الطلبات في نظام إدخال أوامر الأطباء المحوسب (CPOE) الخاص بنظام معلومات Fakeeh Care قبل الصرف.
  • ضمان استدعاء المرضى إلى نوافذ الصرف بشكل فوري وفي الوقت المناسب، بهدف تقليل وقت الانتظار إلى أدنى حد ممكن.
  • تقديم المشورة للمرضى ومقدمي الرعاية حول الاستخدام الآمن والسليم للأدوية، والتخزين، والتفاعلات، وغيرها.
  • اتباع جميع إجراءات سلامة الأدوية عند صرفها.
  • الحفاظ على مراقبة المخزون وأمن جميع الأدوية، وحفظ السجلات والتخزين الآمن للمخدرات والأدوية الخاضعة للرقابة.
  • تحديد الأدوية التي تقترب من تاريخ انتهاء الصلاحية والتعامل معها وفقاً لسياسات Fakeeh Care، وعدم صرف أي دواء بعد تاريخ انتهاء صلاحيته.
  • إكمال عملية تحصيل رسوم المرضى مقابل الأدوية المصروفة بشكل صحيح.
  • إظهار المعرفة بالتخزين السليم والتعامل مع المواد الخطرة في مكان العمل، بالإضافة إلى إجراءات الإسعافات الأولية اللازمة للتعرض العرضي.
  • إظهار المعرفة والمهارات في استخدام قائمة الأدوية المعتمدة، التعرف على البدائل العامة، المؤشر السريري، ودور حدود الخصم وتطبيق قيود كمية الوصفات.
  • الممارسة والامتثال الكامل لمعايير مكافحة العدوى والوقاية الحالية عند التعامل مع الأدوية وتحضير محاليل الوريد والتغذية الوريدية الكاملة (TPN).
  • المساهمة في تدريب وتعريف الصيادلة الجدد والطلاب والمتدربين.
  • إظهار المعرفة بالأدوية التي تصرف دون وصفة طبية (OTC) الشائعة.
  • تقديم تقارير إلى مشرف/مدير الصيدلية وتسجيل أي خطأ دوائي أو تفاعل دوائي ضار (ADR) في نظام الإبلاغ.
  • حضور برامج التعليم المستمر للحفاظ على الخبرة في الصيدلة والترخيص المطلوب.
  • ضمان التسجيل في برنامج التعليم المستمر المتعلق بممارسات إدارة الأدوية، الأدوية عالية الخطورة، أخطاء الأدوية وغيرها.
  • تثقيف المرضى وعائلاتهم حول الحالات الصحية والعلاجات والتدابير الوقائية إن أمكن.
  • جميع موظفي Fakeeh Care مسؤولون عن التحسين المستمر، بما في ذلك أداء جميع المهام والواجبات بطريقة تدعم قيم الرعاية المتمحورة حول الشخص، المساهمة الفاعلة في مبادرات التحسين المستمر ضمن نطاق الدور، الالتزام ببروتوكولات السلامة ومعالجة المخاوف المتعلقة بالسلامة الوظيفية، الحفاظ على السرية التامة للمعلومات الحساسة، الامتثال لسياسات الأمن السيبراني، الالتزام بمدونة قواعد السلوك والسياسات والمعايير الأخلاقية، وإكمال التعليم الإلزامي قبل شهر على الأقل من انتهاء صلاحيته.

الشروط والمتطلبات

  • خبرة من 0 إلى 2 سنة في بيئة الصيدلية بالمستشفى.
  • درجة البكالوريوس في الصيدلة.
  • إجادة جيدة للغة الإنجليزية شفوياً وكتابياً.
  • ترخيص مزاولة المهنة من هيئة الرقابة الصحية الإقليمية (مثل SCFHS / DHA).

المهارات المطلوبة

  • مهارات حاسوبية جيدة مع برامج مثل Microsoft Word وExcel.
  • السعي النشط لتطوير المهارات المهنية من خلال التدريب والخبرة المناسبة.

Description

Job Purpose:

Dispenses medications ordered by privileged physicians while observing at all times highest standards of safety and quality and ensuring prompt seamless process that is conducive to highest patient satisfaction.

Key Responsibilities And Duties

  • Reviews, interprets, clarifies doctor’s medications orders and verifies all orders doses, duration and checks for potential drug-to-drug interaction and clinical indication. Communicates with ordering doctor in case of any ambiguity or concern.
  • Oversees the preparation of the verified medications, performs double-checking and review of proper labeling for the prepared medications and then dispenses.
  • Calculates the amount of the drug product to dispense for a prescription based upon body weight and patient age.
  • Performs computer entry of outside prescriptions or orders into CPOE of Fakeeh Care HIS before dispensing.
  • Ensures prompt and timely calling of patients to dispensing windows and aims at reducing patients waiting time to the minimum possible.
  • Counsels the patients and caregivers on the proper & safe use of medication, storage, and interactions, etc.
  • Follows all medication safety procedures when dispensing medications.
  • Maintains inventory control & security of all drugs, record keeping and secured storage of narcotics and controlled medications.
  • Identifies and appropriately handles the nearly expired medications according to Fakeeh Care policies and NEVER dispenses a medication beyond its expiry date.
  • Properly completes charging of patients for dispensed medications.
  • Demonstrates knowledge of proper storage & handling of hazardous materials found in her/him workplace, as well as first aid measures necessary for accidental exposure.
  • Demonstrates knowledge & skills in utilizing formulary, identifications of generic substitution, indication and the role of deductible limits and application of prescription quantity limitations.
  • Practices and fully complies with current infection prevention and control standards when handling medications and preparing IV and TPN formulas.
  • Contributes to training and induction orientation for newly hired pharmacists and students and trainees.
  • Demonstrates knowledge of commonly used over the counter (OTC) drugs.
  • Reports to pharmacy supervisor/ manager and enters on OVR any encountered medication error or ADR.
  • Attends continuing education programs to maintain expertise in pharmacy and required licensure.
  • Ensures Enrollment in continuous education program regarding medication management practices, high risk medications, medication errors and others.
  • Educates patients and their families about health conditions, treatments and preventative measures if applicable.

Requirements

Skills and Abilities:

  • Competent computer skills with software applications to include Microsoft Word, Excel.
  • Actively strive to upgrade professional skills through engaging in appropriate professional training and experience.

Experience

0 – 2 years of experience in hospital pharmacy setting.

Education

Bachelor’s Degree of Pharmacy.

Language

Good command of oral and written English.

Licenses / Certifications

License for practice as per the regional health regulatory authority e.g. (SCFHS / DHA).
