تعلن فقيه للرعاية الصحية عن توفر وظيفة استشاري مسالك بولية في منطقة المدينة المنورة.
المهام والمسؤوليات
- أداء المهام السريرية وفقًا للصلاحيات السريرية الممنوحة له/لها.
- إجراء تقييم شامل للمرضى (مثل التقييم السريري والاجتماعي والاقتصادي والنفسي والوظيفي) وتوثيق التقييم في السجلات الطبية.
- وضع خطة رعاية للمرضى باستخدام منهج قائم على الأدلة ووفقًا لمعايير الرعاية، وتوثيق الخطة في السجل الطبي.
- ضمان تنفيذ خطة رعاية المرضى التي يضعها/تضعها في الوقت المناسب.
- يكون الطبيب المسؤول الرئيسي (MRP) عن المرضى المنومين تحت اسمه/اسمها.
- للمرضى المنومين تحت اسمه/اسمها، وضع خطة خروج من المستشفى في يوم الدخول وإشراك خدمة تخطيط الخروج في أقرب وقت ممكن إذا لزم الأمر.
- تقييم المرضى المنومين تحت اسمه/اسمها مرة واحدة على الأقل يوميًا وضمان توثيق التقييم اليومي وخطة الرعاية في ملاحظات التقدم اليومية باستخدام تنسيق SOAP.
- الرد على الاستشارات الطبية في الوقت المناسب.
- توثيق التدخلات الطبية التي يجريها/تجريها للمرضى في الوقت المناسب.
- اتباع معايير فقيه للرعاية الصحية لتوثيق السجلات الطبية بدقة.
- المسؤولية عن إكمال وتوثيق ملخصات الخروج للمرضى المنومين تحت اسمه/اسمها في الوقت المناسب، والتأكد من تقديم تعليمات الخروج والتوعية بالأدوية المنزلية قبل خروج المريض.
- الالتزام بواجبات منشأة فقيه للرعاية الصحية والاستجابة للمكالمات من المنشأة وفقًا لجدول المناوبات بالقسم/الوحدة.
- حضور اجتماعات القسم والاجتماعات العامة والمساهمة بأفكار لتحسين الكفاءة والإنتاجية ورضا المرضى.
- المسؤولية عن كتابة التقارير الطبية لمرضاه/مرضاها في الوقت المناسب ووفقًا لتنسيق فقيه للرعاية الصحية.
- المساهمة في جولات التدريس والأنشطة التعليمية حسب ما يطلبه رئيس القسم الطبي.
- تثقيف المرضى وعائلاتهم حول الحالات الصحية والعلاجات والإجراءات الوقائية.
- مهام أخرى مكلف بها ضمن نطاق المسؤولية ومتطلبات الوظيفة.
المهارات المطلوبة
- القدرة على التواصل بوضوح مع المرضى وعائلاتهم والأطباء والموظفين غير الطبيين.
- مهارات استخدام برامج مايكروسوفت الحاسوبية.
- مهارات تنظيمية جيدة والقدرة على العمل في حالات الضغط العالي.
الشروط والمتطلبات
- إكمال الخبرة وفقًا لمتطلبات الهيئة التنظيمية الصحية الإقليمية (كما هو مذكور في التعليم) ضمن مجال التخصص في مراكز طبية معترف بها.
- إكمال تدريب منظم في مجال التخصص أو التخصص الفرعي في مركز طبي معترف به، والحصول على أعلى درجة سريرية في ذلك المجال التخصصي التي يمكن أن يمنحها المركز الطبي.
- إجادة ممتازة للغة الإنجليزية والعربية تحدثًا وكتابة.
- رخصة مزاولة المهنة من الهيئة التنظيمية الصحية الإقليمية (مثل الهيئة السعودية للتخصصات الصحية / هيئة الصحة بدبي)، بالإضافة إلى شهادة دعم الحياة الأساسي (BLS) والشهادات المتقدمة إن وجدت.

Description
Skills and Abilities:
Completion of experience according to the regional health regulatory authority (as mentioned in education) within the field of specialty, in recognized medical centers.
Education
Completion of structured training in the field of specialty or subspecialty in a recognized medical center, and obtaining the highest clinical degree in that field of
specialty that can be granted by that medical center.
Language: Excellent command of oral and written English and Arabic.
Licenses / Certifications
License for practice as per the regional health regulatory authority e.g. (SCFHS /
DHA), BLS and advanced license if applicable.
- Performs clinical duties as per his / her assigned clinical privileges.
- Assesses his / her patients thoroughly (eg. clinical, socioeconomic, psychological and functional assessments)
- Develops a plan of care for patients using an evidence-based approach and following standards of care, and
- Ensures that patients’ plan of care that is put by him/her is being implemented in a timely manner.
- Becomes the most responsible physician (MRP) for inpatients admitted under his / her name.
- For inpatients admitted under his / her name, develops discharge plan on day of admission and involves discharge-planning service as early as possible if warranted.
- Assesses inpatients that are admitted under his / her name at least once daily and ensures documentation of daily assessment and plan of care in daily progress notes using SOAP format.
- Responds to medical consultations in timely manner.
- Documents medical interventions that he / she performs on patients in a timely manner.
- Strictly follows Fakeeh care standards for documentation in medical records
- Responsible for timely completion and authentication of discharge summaries for patients admitted under his / her name. Also responsible for ensuring that indicated discharge instructions and education on take home medications is done prior to patients discharge.
- Attends to Fakeeh Health Care facility duties and responds to calls from the facility as per the departmental / section call schedule.
- Attends departmental and general staff meetings and contributes ideas for improving efficiency, productivity and patient satisfaction.
- Responsible for writing medical reports of his / her patients in a timely manner an das per Fakeeh care format
- Contributes to teaching rounds and teaching activities as requested by chairperson of medical department.
- Educates patients and their families about health conditions, treatments and preventative measures.
- Other duties as assigned within the scope of responsibility and requirements of the job.
Skills and Abilities:
- Must be able to clearly communicate with patients and their families, physicians and non-medical staff.
- Microsoft computer skills.
- Should have good organization skills and be able to work in high stress situations.
