نبذة عن الوظيفة
يقدم أخصائي طب الأسرة الأولى رعاية صحية أولية شاملة، حضورياً وعبر الاتصال الطبي عن بُعد، للأفراد والأسر من جميع الأعمار، وتشمل الرعاية الوقائية والحادة والمزمنة. يتولى التشخيص والعلاج لمجموعة واسعة من الحالات الطبية، ونشر التثقيف الصحي والإجراءات الوقائية، وتنسيق الرعاية مع الأخصائيين وسائر الكوادر الصحية. كما يمارس إدارة الصحة العامة للسكان والرعاية القائمة على القيمة، بهدف تحسين نتائج المرضى عبر تقديم رعاية فعّالة وكفؤة من خلال برامج رعاية محددة ومسارات سريرية، مع التركيز على الرعاية المتمحورة حول المريض وتحسين النتائج الصحية.
المهام والمسؤوليات
- إجراء الاستشارات الطبية عن بُعد والحضورية، بما يشمل أخذ التاريخ المرضي الشامل والفحص السريري للمرضى الذين يعانون من شكاوى طب الأسرة والطب العام.
- تقديم الرعاية الصحية لجميع الفئات المستهدفة في طب الأسرة بمختلف الأعمار والأجناس.
- تقديم استشارات ومتابعة الأمراض المزمنة والرعاية الوقائية وفق الإرشادات السريرية.
- استخدام منصات الاتصال المرئي لتقييم أعراض المريض ومراجعة تاريخه الطبي وإجراء الفحوصات الافتراضية، ووضع خطط علاج شخصية وصرف الأدوية وتقديم التثقيف الصحي المستمر.
- تطوير خطط علاج شخصية وتنفيذها، تشمل إدارة الأدوية والإحالة إلى الأخصائيين وتعديلات نمط الحياة.
- الإشراف على الرعاية المستمرة للحالات المزمنة والحادة، وتعديل خطط العلاج بناءً على ملاحظات المريض والنتائج السريرية.
- التعاون مع الكوادر الصحية، ومنهم أطباء الرعاية الأولية والأخصائيون والممرضون، لضمان سلاسة الرعاية المقدمة للمريض.
- ممارسة إدارة الصحة العامة للسكان من خلال تحديد القضايا الصحية على مستوى المجتمع ومعالجتها، وتطبيق الإجراءات الوقائية، وتعزيز التثقيف الصحي والتوعية.
- توثيق النتائج وخطط العلاج بدقة في السجلات الطبية للمرضى.
- التواصل بفعالية مع المرضى وأسرهم، وتقديم شروحات واضحة وموجزة للتشخيصات وخيارات العلاج والمتابعة.

- مراجعة بروتوكولات الامتثال بانتظام، والمشاركة في الدورات التدريبية، والتعاون مع مسؤولي الامتثال لضمان التزام الممارسات بالمعايير التنظيمية.
- إعداد التقارير الخاصة بالمراجعات والتفتيش بدقة وشمول.
- متابعة التغييرات في الأنظمة الصحية وتطبيق التعديلات اللازمة.
- تطوير المعرفة والمهارات المهنية المستمرة في طب الأسرة وممارسات الرعاية الحضورية والاتصال الطبي عن بُعد، وحضور المؤتمرات وورش العمل والدورات الإلكترونية.
- الحفاظ على دقة السجلات الطبية واكتمالها وفقاً للوائح HIPAA.
- إتقان استخدام منصة الاتصال الطبي عن بُعد المعتمدة، بما في ذلك الاتصال المرئي والرسائل الآمنة وربط السجلات الصحية الإلكترونية، والمشاركة في البرامج التدريبية، والتعاون مع الدعم الفني لحل المشكلات التقنية.
- الالتزام بمعايير سلامة المرضى في جميع التفاعلات السريرية، والإبلاغ عن أي مخاطر أو مشكلات تتعلق بسلامة المرضى للجهة المختصة في إدارة الجودة.

- تنظيم دورات تدريبية منتظمة حول سلامة المرضى للكوادر الصحية، واستخدام استبيانات رضا المرضى لجمع الملاحظات بهدف التحسين المستمر.
- العمل مع الفرق متعددة التخصصات لتحسين الرعاية المقدمة للمرضى، ومشاركة الرؤى والتوصيات، وتسهيل التواصل بين أعضاء الفريق بشأن حالة المريض واحتياجاته.
الشروط والمتطلبات
- التدريب الداخلي (Residency) في طب الأسرة من برنامج معتمد.
- الحصول على الزمالة (Board certification) في طب الأسرة.
- التصنيف والتسجيل لدى الهيئة السعودية للتخصصات الصحية (SCFHS) كمساعد استشاري طب الأسرة.
- خبرة مثبتة كمساعد استشاري طب الأسرة أو في دور مماثل، ويفضل في بيئة الرعاية الأولية.
- خبرة في ممارسة إدارة الصحة العامة للسكان ومناهج الرعاية القائمة على القيمة.

Job Description

Patient Consultations and Management
- Conduct comprehensive telehealth consultations and in-person consultations via thorough patient history and physical examinations with patients experiencing Family Medicine and general medicine complaints
- Provide healthcare to all family medicine target population across age and gender
- Provide chronic diseases and preventive care consultations and follow up to patients as per clinical guidelines
- Utilize video conferencing platforms to assess patient symptoms, review medical history, and perform virtual examinations. Develop personalized treatment plans, prescribe medications, and provide ongoing patient education
- Deliver high-quality medical consultations to patients
- Utilize telehealth platforms for virtual appointments and conduct in-person assessments as needed.
- Develop and implement personalized treatment plans, including medication management, referrals to specialists, and lifestyle modifications
- Oversee ongoing care for chronic and acute conditions
- Adjust treatment plans as necessary based on patient feedback and clinical outcomes.
- Collaborate with other healthcare professionals, including primary care physicians, specialists, and nurses, to ensure seamless patient care
- Practice population health management by identifying and addressing health issues at the community level, implementing preventive measures, and promoting health education and awareness

- Experience in practicing population health management and value-based care approaches.
